Summary

Extrapulmonary tuberculosis affecting the hand is rare. Early surgical excision combined with anti-tubercular chemotherapy offers good functional results and prevents recurrence of hand tuberculosis.

Observation:

  • A prospective observational study included patients with suspected primary soft tissue tuberculosis of the hand.
  • Diagnostic methods included hand, wrist, and lung radiographs, and MRI.
  • Diagnosis was confirmed by histology and mycobacterial culture.

Findings:

  • Five patients (3 males, 2 females; mean age 44.75 years) were studied.
  • Two patients had cutaneous hand tuberculosis, and three had synovial sheath involvement.
  • Histology confirmed tuberculosis, and Mycobacterium bovis was identified microbiologically.

Implications:

  • Delayed diagnosis of hand tuberculosis is common due to slow progression and differential diagnoses.
  • Early radical excision of infected tissues coupled with multidrug anti-tubercular therapy is crucial.
  • This combined approach yields good functional outcomes and prevents disease recurrence.
