Brain pathway differences between Parkinson's disease patients with and without REM sleep behavior disorder

Parkinson's disease patients with REM sleep behavior disorder (RBD) show distinct white matter changes. These brain abnormalities correlate with faster motor and cognitive decline in Parkinson's disease.

- Diffusion MRI connectometry, utilizing spin distribution function (SDF), was used for group analysis.
- 23 RBD-positive PD patients were compared with 31 RBD-negative PD patients.

- PD patients with RBD exhibited white matter changes in the cingulum, inferior fronto-occipital fasciculus (IFOF), corticospinal tracts (CST), and middle cerebellar peduncles (MCPs).
- These alterations were identified in both left and right brain hemispheres.
Conclusions:
- White matter changes in the cingulum and IFOF may contribute to faster cognitive decline (visual recognition, visuospatial, executive function) in PD patients with RBD.
- Altered CST structural markers in PD patients with RBD align with their more rapid motor progression.
