    Background:

    • Idiopathic pericarditis is an inflammation of the pericardium of unknown cause.
    • Pregnancy presents unique physiological challenges that can affect pre-existing conditions.

    Observation:

    • This case series describes three patients with idiopathic pericarditis in relation to pregnancy.
    • The condition preceded pregnancy in two patients and emerged during the third trimester in one.

    Findings:

    • All pregnancies resulted in full-term, uneventful deliveries.
    • One patient experienced a slight worsening of mild heart failure during pregnancy; others had no heart failure, arrhythmias, or embolic events.
    • Two patients received aspirin and prednisone, one received heart failure treatment, and all remained symptomatic post-pregnancy, requiring ongoing management.

    Implications:

    • Idiopathic pericarditis can be managed during pregnancy, though long-term symptoms may persist.
    • Further research is needed to understand the chronic implications of idiopathic pericarditis in pregnant individuals.
    • Partial pericardiectomy was performed in two patients, suggesting surgical intervention may be considered for refractory cases.
