Summary

Carotid artery webs are associated with ischemic stroke in patients without other stroke causes. This finding suggests carotid webs may be an underrecognized stroke risk factor.

Background:

  • Cerebral ischemia is a significant cause of morbidity and mortality.
  • The etiology of ischemic stroke remains undetermined in a subset of patients.
  • Carotid artery webs are congenital anomalies that may predispose to stroke.

Purpose of the Study:

  • To investigate the association between carotid artery webs and ischemic stroke.
  • To determine if carotid webs are an independent risk factor for stroke.

Main Methods:

  • A single-center, age- and sex-matched case-control study was conducted.
  • Cases included patients with anterior circulation ischemic stroke of undetermined etiology.
  • Controls included patients with cerebral aneurysms, arteriovenous malformations, or intracerebral hemorrhages.

Main Results:

  • A carotid artery web was present in 9.4% of ischemic stroke cases versus 1.0% of controls (OR=8.0, P=0.032).
  • No significant difference in nonstenotic atherosclerotic plaque was observed between groups.
  • High interobserver agreement was achieved for web diagnosis (Cohen κ=0.93).

Conclusions:

  • Carotid artery webs are significantly associated with ischemic stroke in patients lacking alternative etiologies.
  • Carotid webs represent a potentially underappreciated risk factor for stroke.
  • Further research is warranted to elucidate the pathogenic mechanisms.
