Narcolepsy01:07

Narcolepsy

694
Narcolepsy is a chronic sleep disorder characterized by pervasive, uncontrolled sleepiness and other sleep disturbances. One of its hallmark symptoms is an abrupt transition to REM sleep upon falling asleep, which causes symptoms typically associated with this phase to occur unexpectedly during wakefulness. These include the following symptoms, which typically last from a minute or two to half an hour.

Sleep Apnea01:21

Sleep Apnea

693
Sleep apnea is a condition where breathing stops intermittently during sleep, often leading to significant health issues. Each episode can last from 10 to 20 seconds or more and is frequently accompanied by a brief arousal from sleep. This disturbance, largely unnoticed by the individual, can lead to severe daytime fatigue. Commonly, individuals seek help after being informed by their partners about loud snoring and noticeable breathing pauses during sleep.
The condition is more prevalent among...

The Parathyroid Glands00:59

The Parathyroid Glands

5.6K
The two pairs of parathyroid glands embedded within the posterior surface of the thyroid gland are restricted by a dense capsule around them. These glands comprise two distinct cell populations—parathyroid oxyphil and parathyroid principal cells- pivotal in calcium homeostasis.
Oxyphil cells, whose functions remain elusive, emerge during late puberty, adding a layer of complexity to the parathyroid gland's intricacies. In contrast, principal parathyroid cells undertake a vital role by...

REM Sleep Behavior Disorder01:15

REM Sleep Behavior Disorder

2.2K
REM Sleep Behavior Disorder (RBD) is a sleep disorder characterized by the absence of muscle paralysis that normally occurs during the REM phase of sleep. This absence allows individuals to physically act out their dreams, which are often vivid and disturbing. Common behaviors exhibited during episodes include kicking, punching, and yelling. These actions can be dangerous, potentially leading to injuries for the person with RBD or their bed partner.
RBD is significantly associated with...

Substance Use Disorders Affecting Sleep01:24

Substance Use Disorders Affecting Sleep

507
Substance use disorders involve a pattern of using drugs more extensively than intended and continuing use despite harmful consequences. This includes legal substances like alcohol and nicotine, as well as illegal drugs. These disorders often involve both physical and psychological dependence, reflecting compulsive use of substances that significantly alter thoughts, feelings, and behaviors, contributing to a major public health issue.
Understanding the concepts of physical dependence,...

Insufficient Sleep and Sleep Deprivation01:13

Insufficient Sleep and Sleep Deprivation

1.2K
Insufficient sleep refers to not getting the recommended amount of sleep for optimal functioning, even if it's just slightly less than needed. Sleep insufficiency may occur due to lifestyle choices, such as staying up late for social events or work, resulting in routinely getting less sleep than required. For example, consistently sleeping 6 hours when the body needs 7-9 hours can lead to cumulative effects on health and well-being.
Sleep deprivation is a more severe form of sleep loss...

Summary

Primary hyperparathyroidism significantly impacts sleep quality. Parathyroidectomy improves sleep for over a third of patients, with benefits often seen within one month post-surgery.

Area of Science:

  • Endocrinology
  • Surgical Outcomes
  • Sleep Medicine

Background:

  • Primary hyperparathyroidism (PHPT) is associated with numerous comorbidities, including sleep disturbances.
  • Assessing the impact of parathyroidectomy on sleep quality in PHPT patients is crucial for understanding treatment outcomes.

Purpose of the Study:

  • To evaluate the changes in sleep quality in patients with PHPT following parathyroidectomy.
  • To compare sleep quality improvements in PHPT patients undergoing parathyroidectomy versus control patients undergoing thyroidectomy.

Main Methods:

  • A prospective survey study involving 110 patients with PHPT undergoing parathyroidectomy and 45 controls undergoing thyroidectomy.
  • The Pittsburgh Sleep Quality Index (PSQI) was administered preoperatively and at 1 and 6 months postoperatively.
  • Poor sleep quality was defined as a PSQI score >5; a clinically significant improvement was a ≥3-point decrease.

Main Results:

  • Preoperatively, PHPT patients reported significantly worse sleep quality than controls (mean PSQI 8.1 vs 5.3).
  • Postoperatively, parathyroidectomy patients showed significant improvement in sleep quality, with 37% experiencing a clinically important improvement at 1 month and 40% at 6 months.
  • No significant difference in the proportion of patients with poor sleep quality was observed between groups at 1 or 6 months postoperatively.

Conclusions:

  • Over two-thirds of patients with PHPT experience poor sleep quality.
  • Parathyroidectomy leads to significant improvements in sleep quality for over one-third of PHPT patients.
  • Sleep quality improvements are typically observed within the first month after parathyroidectomy.
