Area of Science:

  • Neuroimaging
  • Neurosurgery
  • Neuroscience

Background:

  • Deep brain stimulation (DBS) targets white matter tracts for treating conditions like treatment-resistant depression (TRD).
  • The stria medullaris thalami (SM) is a critical afferent pathway to the lateral habenula (LHb), implicated in TRD pathophysiology.

Purpose of the Study:

  • To determine if diffusion tensor imaging (DTI) can effectively visualize the SM for DBS targeting.
  • To assess the reliability of SM visualization using DTI in patients undergoing DBS surgery.

Main Methods:

  • Probabilistic DTI was performed on ten cerebral hemispheres from five patients pre-DBS surgery.
  • Tractography initiated from manual identification of the lateral habenula (LHb) on MRI.
  • Variations in seed region placement within the LHb were assessed for tractography impact.

Main Results:

  • The stria medullaris thalami (SM) was consistently visualized across all hemispheres.
  • Tractography of the SM showed minimal significant changes despite variations in seed slice or voxel placement within the LHb.

Conclusions:

  • Probabilistic DTI is a reliable method for visualizing the stria medullaris thalami (SM).
  • This technique holds potential for direct anatomical targeting in deep brain stimulation (DBS) surgery for conditions like TRD.
