Summary

Collecting Duct Carcinoma (CDC), a rare kidney cancer, can present with Inferior Vena Cava (IVC) thrombus and retroperitoneal lymph nodes. This case highlights the aggressive nature of Bellini duct carcinoma and its management.

Background:

Collecting Duct Carcinoma (CDC), also known as Bellini duct carcinoma, is a rare and aggressive kidney malignancy.
It originates from the collecting ducts of the kidney, often presenting with a poor prognosis.

Observation:

This report details a unique case of CDC with Inferior Vena Cava (IVC) thrombus.
The case also involved large retroperitoneal lymph nodes and a diffuse desmoplastic reaction.

Findings:

Morphological and immunohistochemical analyses confirmed the diagnosis of Bellini duct carcinoma.
The patient underwent surgical intervention including radical nephrectomy with IVC thrombectomy and lymphadenectomy.

Implications:

This case underscores the importance of considering CDC in the differential diagnosis of aggressive renal tumors.
Adjuvant chemotherapy with gemcitabine and cisplatin was initiated, highlighting potential treatment strategies.
Further research into the management of advanced CDC with IVC involvement is warranted.
