Summary

Single-session Gamma Knife radiosurgery offers a safe and effective treatment for optic pathway/hypothalamic gliomas. This study shows a 90% tumor control rate and preserved or improved vision in most patients, indicating promising outcomes for this radiosurgery approach.

Background:

  • Optic pathway/hypothalamic gliomas require careful irradiation due to their critical location.
  • Stereotactic fractionated radiotherapy is often preferred for safety in this region.

Purpose of the Study:

  • To evaluate the safety and efficacy of single-session stereotactic radiosurgery for optic pathway/hypothalamic gliomas.

Main Methods:

  • 22 patients with optic pathway/hypothalamic gliomas underwent single-session Gamma Knife radiosurgery (December 2004 - June 2014).
  • Follow-up data for at least 1 year was available for 20 patients.
  • Tumor volumes ranged from 0.15 to 18.2 cm³ with prescription doses of 8–14 Gy.

Main Results:

  • A 90% tumor control rate was observed (12 shrinkage, 6 stable).
  • Vision remained stable or improved in 18 out of 20 patients.
  • Progression-free survival was 83% at 3 years.

Conclusions:

  • Single-session Gamma Knife radiosurgery demonstrates initial safety and efficacy for treating optic pathway/hypothalamic gliomas.
  • This approach represents a viable alternative for managing these challenging tumors.
