Aneurysm III: Interprofessional Care

425
Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. Area of Science:

  • Gastroenterology
  • Vascular Surgery
  • Minimally Invasive Surgery

Background:

  • Splenic artery aneurysm (SAA) is a rare condition with potentially fatal complications.
  • Historically, treatment involved spleen excision; however, spleen-preserving techniques are now preferred.

Purpose of the Study:

  • To evaluate the feasibility and outcomes of laparoscopic aneurysmectomy with spleen preservation for splenic artery aneurysms.

Main Methods:

  • Analysis of prospectively collected data from patients undergoing laparoscopic surgery for splenic diseases between 1998 and 2016.
  • Inclusion criteria focused on attempted laparoscopic aneurysmectomy with the goal of spleen salvage.

Main Results:

  • Seven out of eleven patients successfully underwent aneurysmectomy with spleen preservation.
  • One patient had a partial splenectomy, two had intra-operative splenectomies, and one required re-operation with splenectomy due to complications.

Conclusions:

  • Laparoscopic aneurysmectomy for splenic artery aneurysms appears to be a safe and effective treatment.
  • This method allows for definitive SAA treatment while preserving spleen function, though larger studies are needed.
