Summary

Background:

  • Pyrazinamide (PZA) is a crucial first-line drug for tuberculosis treatment.
  • Accurate susceptibility testing is vital for effective tuberculosis management and preventing drug resistance.
  • The BACTEC MGIT 960 (M960) system is widely used for Mycobacterium tuberculosis susceptibility testing.

Purpose of the Study:

  • To assess the susceptibility of Mycobacterium tuberculosis complex isolates to pyrazinamide (PZA).
  • To compare different testing methods for PZA susceptibility, including critical concentration and reduced inoculum.
  • To investigate the correlation between PZA resistance and pncA mutations.

Main Methods:

  • 253 Mycobacterium tuberculosis complex isolates were tested for PZA susceptibility using the BACTEC MGIT 960 (M960) system.
  • Resistant isolates underwent repeat testing with a critical concentration (PZA-200) and a reduced inoculum.
  • Susceptibility was also determined using the BACTEC 460 (B460) reference method, and pncA gene sequencing was performed.

Main Results:

  • Out of 253 isolates, 37 showed resistance to PZA on the M960 system.
  • The reduced inoculum method demonstrated higher agreement with reference methods and pncA mutation analysis compared to the critical concentration (PZA-200) assay.

Conclusions:

  • This method shows better concordance with established reference assays and genetic analysis of pncA.
  • Accurate PZA susceptibility testing is essential for guiding tuberculosis treatment regimens.
