Purpose of the Study:

  • To investigate bronchodilator response patterns in advanced lung function parameters.
  • To analyze these patterns continuously relative to forced expiratory volume in 1 second (FEV1) percent predicted (%p) in COPD patients and controls.

Main Methods:

  • Eighty-one smokers/ex-smokers (41 controls, 40 COPD) underwent spirometry, body plethysmography, impulse oscillometry, and single-breath carbon monoxide diffusion.
  • Measurements were taken at baseline and after sequential salbutamol and ipratropium inhalation.

Main Results:

  • Most parameters showed a linear response to decreasing FEV1%p.
  • Inspiratory capacity (IC), forced vital capacity (FVC), FEV1/FVC, and expiratory resistance (Rex) exhibited segmented responses.
  • Volume parameter responses did not correlate with FEV1%p response in different FEV1%p groups.

Conclusions:

  • Advanced lung function parameter responses to bronchodilators vary based on baseline FEV1%p.
  • Volume parameter responses are most pronounced in COPD patients with FEV1%p <65.
  • Volume and resistance responses offer complementary information to FEV1 reversibility for identifying responders.
