Background:

  • Interdural dermoid cysts (DCs) within the cavernous sinus (CS) are exceptionally rare pediatric central nervous system lesions.
  • These cysts are typically located between the dural and membranous layers of the CS lateral wall.

Observation:

  • A 5-year-old boy presented with third cranial nerve palsy and exophthalmos, indicative of a space-occupying lesion.
  • Imaging revealed an interdural dermoid cyst of the right cavernous sinus.

Findings:

  • The patient underwent successful gross-total surgical removal of the interdural DC via a frontotemporal approach.
  • Post-operative follow-up at 12 months showed no evidence of recurrence and a good clinical outcome.

Implications:

  • This case contributes to the limited literature on pediatric interdural cavernous sinus dermoid cysts.
  • It underscores the feasibility and efficacy of surgical intervention for these rare pediatric lesions.
