    Summary

    Schistosomiasis-associated arthritis can be diagnosed using specific criteria. Treatment with praziquantel led to significant improvement in 90% of patients with schistosomal arthritis.

    Background:

    • Schistosomiasis is a parasitic disease that can manifest with joint involvement.
    • Arthritis in schistosomiasis patients requires specific diagnostic criteria to differentiate from other arthropathies.

    Purpose of the Study:

    • To establish diagnostic criteria for arthritis associated with schistosomiasis.
    • To evaluate the efficacy of praziquantel in treating schistosomal arthritis.

    Main Methods:

    • 180 cases were classified into four groups: schistosomal arthritic, schistosomal non-arthritic, arthritic non-schistosomal, and controls.
    • Diagnostic tests included Erythrocyte Sedimentation Rate (ESR), Rose-Waaler (RW) for rheumatoid factor (RF), antinuclear antibody (ANA), serum uric acid (SUA), intradermal test (IDT), indirect haemagglutination test (IHAT), and circumoval precipitin test (COPT).

  • 20 patients with schistosomal arthritis received praziquantel and were followed up for 6 months.

    • Main Results:

    • Bilharzial ova were detected in excreta of schistosomal groups.
    • High positivity rates for IDT (90.8%), IHAT (90%), and COPT (88.3%) were observed in schistosomal arthritis patients.
    • RF and ANA positivity were significantly lower in schistosomal arthritis compared to non-schistosomal arthritis.
    • 90% of patients treated with praziquantel showed marked improvement in arthritic symptoms.

    Conclusions:

    • The study suggests criteria for diagnosing schistosomiasis-associated arthritis.
    • Praziquantel is an effective treatment for schistosomal arthritis, leading to significant clinical improvement.
