Summary

Radio-lunate arthrodesis shows promising results for rheumatoid arthritis patients with localized radio-lunate joint arthritis. However, continued carpal degeneration necessitates long-term follow-up before widespread recommendation.

Area of Science:

  • Orthopedic Surgery
  • Rheumatology
  • Hand Surgery

Background:

  • Rheumatoid arthritis frequently affects the wrist, leading to localized arthritis around the radio-lunate joint.
  • Surgical intervention, such as radio-lunate arthrodesis, is considered for managing this condition.

Purpose of the Study:

  • To evaluate the outcomes of radio-lunate arthrodesis in patients with rheumatoid arthritis affecting the radio-lunate joint.
  • To assess the efficacy and potential complications of this surgical procedure.

Main Methods:

  • Radio-lunate arthrodesis was performed on 15 rheumatoid arthritis patients with localized radio-lunate joint arthritis.
  • Six patients underwent the procedure with the insertion of a Stanley-Shelley plate.
  • Outcomes were assessed 1 to 3.5 years post-operation.

Main Results:

  • Excellent results were observed in nine wrists, good in three, fair in one, and poor in three wrists.
  • Postoperative radiographic assessment revealed continued carpal degeneration in 12 out of 15 wrists.

Conclusions:

  • Radio-lunate arthrodesis can provide satisfactory short-to-medium term outcomes for select rheumatoid arthritis patients.
  • The high incidence of continued carpal degeneration highlights the need for long-term follow-up studies.
  • Further research is required to determine the long-term efficacy and general applicability of radio-lunate arthrodesis.
