[Achlorhydria and stomach carcinoma].

    Summary

    Patients with achlorhydria (lack of stomach acid) face a significantly higher risk of gastric cancer. This study found a 4.7 times increased relative risk, though screening is not recommended.

    Context:

    • Achlorhydria is a condition characterized by the absence of stomach acid.
    • Previous studies report varying risks of gastric adenocarcinoma in patients with achlorhydria.
    • Data on this risk in The Netherlands were previously unavailable.

    Purpose:

    • To investigate the risk of developing gastric adenocarcinoma in achlorhydric patients in The Netherlands.
    • To quantify the relative risk of gastric cancer in this patient group.

    Summary:

    • A cohort of 129 achlorhydric patients was followed for a mean of 12.3 years.
    • Six cases of gastric adenocarcinoma were diagnosed, compared to an expected incidence of 1.275.
    • This resulted in a statistically significant relative risk of 4.7 (95% CI: 1.7-10.2; p<0.05).

    Impact:

    • This study quantifies the elevated gastric cancer risk for achlorhydric individuals in a Dutch population.
    • Despite the increased risk, current findings do not support routine gastric cancer screening for achlorhydric patients.
    • Highlights the need for further research into risk stratification and potential interventions for this population.
