Summary

Laparoscopic liver resection (LLR) is safe for minor resections, with low morbidity and no mortality in this Japanese multicenter study. Further research is needed to confirm safety for major LLR procedures.

Background:

  • Laparoscopic liver resection (LLR) is increasingly adopted globally.
  • Previous studies on LLR safety have been limited, often lacking multicenter prospective data.
  • This study addresses the need for comprehensive data on the current status and safety of LLR in Japan.

Purpose of the Study:

  • To evaluate the safety and current status of laparoscopic liver resection (LLR) in Japan.
  • To assess surgical factors and short-term/midterm outcomes of LLR.
  • To provide data for the safe dissemination of LLR techniques.

Main Methods:

  • A multicenter prospective cohort study involving 12 institutions in Japan.
  • 102 patients undergoing LLR were enrolled from April 2015 to March 2016.
  • Surgical safety was the primary endpoint, assessed via surgical factors and patient outcomes.

Main Results:

  • 96% of procedures were pure laparoscopy; most were minor resections (non-anatomical partial or left lateral sectionectomy).
  • Low median blood loss (80.5 mL) and conversion rate (3.9%) were observed.
  • Postoperative morbidity (Clavien-Dindo grade > II) was 5.9%, with no reoperations or mortality within 90 days.

Conclusions:

  • Minor laparoscopic liver resections are performed safely in Japan.
  • The study provides valuable data on current LLR practices and outcomes.
  • Larger prospective studies are necessary to establish the safety of major LLR.
