Background:

Myeloid sarcoma is an uncommon extramedullary tumor that can manifest independently or alongside acute myeloid leukemia.
Orbital involvement is rare, particularly in infants without pre-existing hematologic malignancy.

An 8-month-old male infant presented with a rapidly growing left orbital mass initially misdiagnosed as infantile hemangioma.
Magnetic resonance imaging revealed a solid, enhancing orbital mass.
