Background:

  • Glaucoma is a progressive optic neuropathy characterized by damage to the optic nerve head.
  • Early detection and monitoring of glaucoma are crucial for preserving vision.
  • The lamina cribrosa (LC) is a key anatomical structure within the optic nerve head affected in glaucoma.

Purpose of the Study:

  • To compare lamina cribrosa (LC) intensity between glaucoma-suspect eyes and eyes with mild to moderate glaucoma.
  • To investigate the potential of LC intensity as a novel biomarker for glaucoma detection and progression.

Main Methods:

  • Swept-source optical coherence tomography (SS-OCT) was used to acquire optic disc volume scans.
  • LC intensity was quantified using Image J software and normalized to the retinal pigment epithelium and vitreous signal.
  • The study included 19 glaucoma-suspect eyes and 29 eyes with mild to moderate glaucoma.

Main Results:

  • Glaucoma-suspect eyes exhibited significantly higher normalized LC intensity (median 0.96 units) compared to mild to moderate glaucoma eyes (median 0.86 units) (P<0.001).
  • A weak positive correlation was observed between mean deviation and normalized LC intensity (r=0.344; P=0.018), suggesting a link to visual field status.
  • No significant difference in mean age was noted between the two groups (P=0.161).

Conclusions:

  • Normalized LC intensity differs significantly between glaucoma-suspect and mild to moderate glaucoma eyes.
  • LC intensity measurement represents a promising novel parameter for glaucoma assessment.
  • Further research is warranted to validate LC intensity as a diagnostic and prognostic tool in glaucoma.
