Summary

A 93-year-old woman presented with acute fever, wrist pain, and swelling. Diagnosis revealed calcium pyrophosphate dihydrate crystal deposition in the wrist.

Area of Science:

  • Rheumatology
  • Crystal-induced arthritis

Background:

  • Crystal-induced arthritis can present with acute symptoms mimicking infectious processes.
  • Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is a common form of inflammatory arthritis.

Observation:

  • A 93-year-old female experienced sudden onset fever, left wrist pain, and swelling.
  • Examination showed symmetric digit swelling, partial flexion, flexor tendon sheath tenderness, and pain on passive extension.

Findings:

  • Synovial fluid analysis revealed the presence of calcium pyrophosphate dihydrate crystals.
  • This finding confirmed a diagnosis of acute calcium pyrophosphate deposition disease.

Implications:

  • Prompt diagnosis and management of CPPD are crucial to prevent joint damage.
