Pleural Effusion Developing in Two Patients on Continuous Ambulatory Peritoneal Dialysis

Pleural effusions in patients on continuous ambulatory peritoneal dialysis (CAPD) can signal a pleuroperitoneal leak. Measuring pleural fluid glucose is a simple diagnostic method for this complication.

- The high glucose content indicated translocation of peritoneal dialysate into the pleural space via a pleuroperitoneal connection.
- One patient resolved effusion with hemodialysis; another improved with automated peritoneal dialysis using smaller volumes.
Implications:
- Pleuroperitoneal leak must be considered in CAPD patients with pleural effusion.
- Pleural fluid glucose measurement is a simple, reliable diagnostic tool for pleuroperitoneal communication.
