Summary

Coronary fractional flow reserve (FFR) did not correlate with noninvasive ischemia tests in patients with severe coronary artery obstructions. This finding suggests limitations in using FFR alone to assess ischemia severity in these cases.

  • Noninvasive methods like stress echocardiography and nuclear medicine are used to detect ischemia.

Purpose of the Study:

  • To evaluate the correlation between FFR and noninvasive ischemia findings.
  • To assess the relationship between FFR and critical coronary artery obstruction.

Main Methods:

  • Retrospective study of 96 patients (March 2011-August 2014) with 107 critical coronary obstructions.
  • Quantitative coronary angiography (QCA) and intracoronary ultrasound (ICUS) estimated obstruction severity.
  • All patients had ischemia confirmed by noninvasive tests.

Main Results:

  • 100% of patients showed ischemia on noninvasive tests.
  • 52% of cases had FFR values ≤ 0.80 with adenosine.
  • No significant correlation was found between FFR ≤ 0.80 and stenosis degree, length, or presence of ischemia.

Conclusions:

  • Coronary FFR using a cut-off of 0.80 showed no correlation with noninvasive ischemia tests.
  • This lack of correlation was observed in patients with severe coronary artery obstructions identified by QCA and ICUS.
