Background:

  • Hereditary diffuse gastric cancer (HDGC) is linked to germline mutations in the CDH1 gene, which encodes E-cadherin.
  • These mutations are associated with early-onset gastric cancer, with a mean diagnosis age of 37 years.

Observation:

  • Individuals with CDH1 mutations face a high lifetime risk of gastric cancer: 67% for men and 83% for women by age 80.
  • Current endoscopic surveillance methods appear largely ineffective for detecting cancer in HDGC mutation carriers.

Findings:

  • Germline CDH1 mutations are a significant risk factor for developing diffuse gastric cancer.
  • The lifetime risk of gastric cancer is substantially elevated in carriers of these mutations.

Implications:

  • Prophylactic total gastrectomy is a recommended preventative measure for individuals identified with germline CDH1 mutations.
  • Surgical intervention should be considered between the ages of 18 and 40 years for mutation carriers to mitigate cancer risk.
