Background:

  • Prior research established the efficacy of the Sadness Program, an Australian technician-assisted Internet-based cognitive behavioral therapy (iCBT) for major depressive disorder (MDD).
  • This study adapted the Sadness Program for an American population and evaluated its effectiveness against an attention control group.

Purpose of the Study:

  • To adapt and test the efficacy of the technician-assisted Sadness Program for treating major depressive disorder (MDD) in an American adult population.
  • To compare the outcomes of iCBT with a monitored attention control (MAC) group.

Main Methods:

  • A parallel-group, randomized controlled trial involving 77 adult MDD participants (18-45 years).
  • Participants were randomized to a 10-week iCBT program (n=37) or MAC (n=40), with weekly technician support for the iCBT group.
  • Primary outcome: Hamilton Rating Scale for Depression (HRSD); Secondary outcomes: Patient Health Questionnaire-9 (PHQ-9) and Kessler-10 (K10).

Main Results:

  • Intent-to-treat analyses showed significantly greater reductions in depressive symptoms for the iCBT group compared to the MAC group (d = -0.80) on both self-report and clinician-rated measures.

Conclusions:

  • The Sadness Program effectively reduced depression and distress symptoms in adults with MDD.
  • Technician-assisted iCBT presents a promising, scalable, and cost-efficient strategy for improving access to mental healthcare.
