Summary
A rare, massive desmoid tumor was found in the pleura of a Pakistani male. Surgical excision was successful, with no recurrence after nine months of follow-up.

Background:
- A 49-year-old male with a history of pleural effusion presented with a massive pleural-based opacity.
- Previous investigations including pleural biopsy and bronchial washings were non-contributory.
- Empirical anti-tuberculosis treatment was administered without symptom resolution.
Observation:
- Chest imaging revealed a large left-sided pleural-based opacity.
- Video-assisted thoracoscopic surgery identified a 2.2 kg mass involving the anterior mediastinum, chest wall, and visceral pleura.
- Conversion to open thoracotomy was necessary for complete mass excision.
Findings:
- Histopathology and immunohistochemistry confirmed the mass as a desmoid tumor.
- The tumor was predominantly pleural-based, a rare presentation.
- Complete surgical excision was achieved.
Implications:
- Desmoid tumors should be considered in the differential diagnosis of pleural masses, especially spindle cell tumors.
- This case highlights the importance of comprehensive histopathological evaluation for rare thoracic neoplasms.
