Background:

  • Transbronchial needle aspiration (TBNA) is crucial for mediastinal staging but can lack accuracy.

Purpose of the Study:

  • To introduce and evaluate a novel virtual bronchoscopy (VB) tool designed to enhance the accuracy of traditional TBNA for mediastinal lymph node staging.

Main Methods:

  • Chest CT images were used to create VB simulations for real-time biopsy guidance.
  • Patients undergoing TBNA were retrospectively divided into traditional and VB-guided groups.
  • Diagnostic yields, including sensitivity and accuracy, were compared between the two groups.

Main Results:

  • The VB group showed improved sensitivity (84.31% vs. 66.6%) and diagnostic accuracy (84.91% vs. 67.92%) compared to the traditional group.
  • VB guidance significantly enhanced the accuracy of sampling paratracheal lymph nodes (P = 0.011).
  • No significant difference was observed for subcarinal lymph node sampling.

Conclusions:

  • The developed VB tool is simple, cost-effective, and readily available.
  • Real-time guidance with VB improves TBNA accuracy, especially for challenging paratracheal lesions.
  • This tool offers a valuable enhancement for mediastinal staging procedures.
