Ocular tuberculosis (TB) can manifest as a scleral abscess with choroidal detachment. Prompt diagnosis and antitubercular treatment are crucial for preserving vision in TB-endemic regions.

Background:

Ocular tuberculosis is a rare but serious condition.
Scleral abscesses typically have bacterial or fungal origins.

Observation:

A 60-year-old woman presented with acute symptoms of scleritis and decreased vision in her right eye.
Examination revealed a scleral abscess with associated choroidal detachment.

Findings:

Scleral biopsy confirmed Mycobacterium tuberculosis via Ziehl-Neelsen staining and PCR.
The patient responded well to antitubercular therapy.

Implications:

Tuberculosis should be considered in the differential diagnosis of scleral abscesses, especially in endemic areas.
Early detection and treatment of ocular TB can lead to favorable visual outcomes.
