Background:

Primary cutaneous T-cell lymphoma (CTCL) gamma-delta subtype is exceptionally rare.
CTCLs represent a diverse group of non-Hodgkin lymphomas affecting the skin.

Observation:

A 66-year-old Malay man presented with rapidly progressing skin nodules and plaques.
Initial combination chemotherapy (gemcitabine, etoposide, carboplatin) achieved near complete remission.

Findings:

The patient relapsed within a month and progressed despite the initial regimen.
Salvage chemotherapy (fludarabine, cytarabine, vinblastine) was ineffective, with subsequent brain metastasis and death.

Implications:

This case highlights the aggressive nature and challenging treatment of CTCL gamma-delta subtype.
Further research and a rare lymphoma registry are needed to improve understanding and treatment strategies.
