Background:

  • Primary cutaneous T-cell lymphoma (CTCL) gamma-delta subtype is exceptionally rare.
  • CTCLs represent a diverse group of non-Hodgkin lymphomas affecting the skin.

Observation:

  • A 66-year-old Malay man presented with rapidly progressing skin nodules and plaques.
  • Initial combination chemotherapy (gemcitabine, etoposide, carboplatin) achieved near complete remission.

Findings:

  • The patient relapsed within a month and progressed despite the initial regimen.
  • Salvage chemotherapy (fludarabine, cytarabine, vinblastine) was ineffective, with subsequent brain metastasis and death.

Implications:

  • This case highlights the aggressive nature and challenging treatment of CTCL gamma-delta subtype.
  • Further research and a rare lymphoma registry are needed to improve understanding and treatment strategies.
