Background:

  • Solid breast masses require accurate diagnostic methods for timely and appropriate treatment.
  • Conventional ultrasound (US) has limitations in characterizing breast lesions.
  • Shear wave elastography (SWE) offers quantitative and qualitative assessments of tissue stiffness.

Purpose of the Study:

  • To evaluate the effectiveness of a novel qualitative shear wave elastography (SWE) classification (Qual1) for solid breast masses.
  • To compare the diagnostic performance of Qual1 with conventional US, quantitative SWE parameters, and a previously proposed qualitative SWE classification (Qual2).

Main Methods:

  • A prospective study enrolled 314 women with 325 breast masses undergoing biopsy.
  • Conventional US and SWE were performed, with masses classified using two qualitative methods: Qual1 (five color patterns) and Qual2 (four-color overlay).
  • Diagnostic performance was assessed by comparing imaging findings with pathological results, including Breast Imaging Reporting and Data System (BI-RADS) assessments and quantitative SWE parameters.

Main Results:

  • The Qual1 classification demonstrated a significantly higher Az value than Qual2 and quantitative SWE parameters.
  • Applying Qual1 for downgrading (Color pattern 1) and upgrading (Color pattern 5) BI-RADS categories yielded the highest Az value (0.951).
  • Qual1 achieved superior specificity (86.56%) compared to conventional US (81.18%) while maintaining high sensitivity (94.96%).

Conclusions:

  • The proposed qualitative classification (Qual1) for SWE is effective in evaluating solid breast masses.
  • Qual1 shows potential as a valuable tool to assist clinicians in breast mass diagnosis.
  • This qualitative approach may represent SWE parameters and enhance diagnostic accuracy.
