Summary

A corneal transplant (penetrating keratoplasty) treated silicone keratopathy after silicone oil injection for retinal detachment. The cornea showed severe endothelial cell loss and damage post-injection.

Background:

  • Pars plana vitrectomy with intravitreal silicone injection is a treatment for complex retinal detachments.
  • Silicone oil can cause complications, including corneal endothelial toxicity.

Observation:

  • A 50-year-old male developed diffuse silicone keratopathy two years post-vitrectomy and silicone oil injection.
  • Specular microscopy showed progressive endothelial cell loss, from 26% at 2 weeks to 69% at 5 months.
  • A silicone droplet was observed in the anterior chamber.

Findings:

  • Penetrating keratoplasty was performed to address the silicone keratopathy.
  • Scanning electron microscopy of the corneal button revealed a damaged posterior surface with fibroblast-like cells and absent endothelial cells.

  • This case highlights the potential for delayed corneal endothelial decompensation following intravitreal silicone oil injection.
  • Corneal complications should be monitored in patients with prior silicone oil use.
