    Background:

    Purpose of the Study:

    • To compare the clinical outcomes of immediate implant loading versus delayed loading with closed healing in the mandibular midline.
    • To analyze patient recruitment and identify key exclusion criteria for this implant therapy study.

    Main Methods:

    • A multicenter study involving nine German universities recruited patients for mandibular implant placement.
    • Inclusion criteria included denture satisfaction, psychological assessment, and adequate posterior mandibular bone height.

    Main Results:

    • Over an extended period, 224 patients were screened, and 169 were included in the trial.
    • The main reasons for exclusion were insufficient residual bone height (15.2%) and psychological factors (1.8%).

    Conclusions:

    • Residual bone height is the most critical factor for selecting elderly edentulous patients for mandibular implant therapy.
    • Psychological assessments were less significant in patient selection compared to bone height.
