Background:

  • Current diagnostic methods for inflammatory arthritis lack specificity, hindering accurate patient classification.
  • There is a critical need for novel biomarkers to differentiate between various forms of arthritis and inflammatory conditions.

Purpose of the Study:

  • To identify specific urinary proteomic biomarkers for rheumatoid arthritis (RA), psoriatic arthritis (PsA), osteoarthritis (OA), and inflammatory bowel disease (IBD).
  • To develop and validate classifiers based on these biomarkers for differential diagnosis.

Main Methods:

  • A study cohort included 50 subjects each with RA, PsA, OA, IBD, and 50 healthy controls.
  • Subjects were divided into training (two-thirds) and validation (one-third) sets.
  • Sequential comparison and classifier development were performed, followed by blind testing on the validation set.

Main Results:

  • Multiple urinary peptides significantly associated with distinct pathological conditions were identified.
  • Developed classifiers demonstrated excellent performance in the validation set, with AUCs ranging from 0.90 to 0.97.
  • Identified peptide markers indicated dysregulation in collagen synthesis, inflammation, and novel inflammatory pathways.

Conclusions:

  • Urinary peptide signatures can reliably differentiate between chronic arthropathies and inflammatory conditions with distinct pathogenesis.
  • This approach offers a promising avenue for improving diagnostic accuracy and patient stratification in rheumatology and gastroenterology.
