Summary

Programmed death-ligand 1 (PD-L1) expression varies across thyroid cancer types. High PD-L1 expression was observed in advanced follicular and anaplastic thyroid carcinomas, suggesting potential therapeutic relevance for refractory cases.

Area of Science:

  • Oncology
  • Immunology
  • Pathology

Background:

  • Programmed death-ligand 1 (PD-L1) expression is a potential biomarker for anti-PD-1/anti-PD-L1 therapy response.
  • Reported PD-L1 positivity frequencies in thyroid carcinomas are variable.
  • Understanding PD-L1 expression patterns is crucial for targeted therapies in thyroid cancer.

Purpose of the Study:

  • To evaluate PD-L1 expression across various thyroid cancer types on a large scale.
  • To analyze the relationship between PD-L1 expression and clinicopathologic factors.
  • To determine the therapeutic relevance of PD-L1 in refractory thyroid cancer.

Main Methods:

  • Analysis of 407 primary thyroid cancers with a median follow-up of 13.7 years.
  • Evaluation of PD-L1 expression using a rabbit monoclonal antibody (clone SP142).
  • Assessment of associations between PD-L1 expression, clinicopathologic variables, disease progression, and oncogenic mutations (TERT promoter, BRAF).

Main Results:

  • Tumoral PD-L1 expression: 6.1% in papillary, 7.6% in follicular, and 22.2% in anaplastic thyroid carcinomas.
  • PD-L1 positivity distribution differed significantly by histology (P < 0.001).
  • PD-L1 was highly expressed in a subset of advanced follicular and anaplastic thyroid carcinomas, with strong intensity in all positive follicular and anaplastic cases.

Conclusions:

  • PD-L1 expression is significantly associated with thyroid cancer histology.
  • High PD-L1 expression in advanced follicular and anaplastic thyroid carcinomas suggests potential for immunotherapy.
  • PD-L1 evaluation may offer therapeutic options for patients with refractory thyroid cancer.
