Cardiac surgery in patients aged 75 and older presents higher mortality risks. However, a standardized perioperative care protocol effectively reduced mechanical ventilation duration and postoperative delirium in this elderly group.
Area of Science:
- Geriatric Medicine
- Cardiovascular Surgery
- Anesthesiology
Background:
- Aging leads to progressive cellular degeneration, impairing physiological responses to surgical stress in elderly patients (≥ 75 years).
- Older adults have a narrower safety margin during surgery, increasing risks of perioperative complications.
Purpose of the Study:
- To compare perioperative complications in patients aged 75 years and older undergoing cardiac surgery with those in younger patients.
- To evaluate the impact of a standardized perioperative care protocol on outcomes in elderly cardiac surgery patients.
Main Methods:
- A retrospective study involving 1446 patients at the Silesian Centre for Heart Diseases (2009-2014).
- Implementation of a standardized perioperative care protocol for seniors to minimize complications, mechanical ventilation, and delirium.
- Comparison of outcomes between patients aged ≥ 75 years and younger patients.
Main Results:
- The mean duration of mechanical ventilation was comparable between elderly (≥ 75 years) and younger patients (13.8 hours).
- In-hospital mortality was significantly higher in the elderly group (3.8%) compared to younger patients.
- Elderly patients undergoing cardiac surgery exhibited more comorbidities, impacting hospital stay duration and mortality.
Conclusions:
- The standardized perioperative care protocol successfully reduced mechanical ventilation duration and postoperative delirium in elderly patients (≥ 75 years).
- Despite improved ventilation and delirium rates, elderly cardiac surgery patients face significantly higher in-hospital mortality due to increased comorbidities.
