Background:

  • Squamoid eccrine ductal carcinoma (SEDC) is a rare eccrine carcinoma subtype.
  • Fewer than thirteen cases have been documented globally.
  • Histopathological diagnosis is challenging, often leading to misdiagnosis.

Observation:

  • A 72-year-old female presented with nasal erythematous, keratotic, and ulcerated papules.
  • Initial incisional biopsy diagnosed SEDC.
  • Multiple surgical interventions and radiotherapy were performed due to positive margins and recurrence.

Findings:

  • The patient experienced positive surgical margins after initial excision and subsequent wide excision with grafting.
  • The lesion recurred after radiotherapy and a further surgical attempt.
  • Persistent disease was confirmed even when intraoperative biopsies appeared clear.

Implications:

  • Accurate distinction between SEDC and squamous cell carcinoma is crucial.
  • SEDC exhibits a more aggressive behavior than squamous cell carcinoma.
  • Wider surgical margins are essential for managing SEDC to prevent recurrence.
