    Background:

    • Pediatric cardiopulmonary arrest affects 16,000 children annually, with 2% of pediatric emergency medical services (EMS) calls attributed to these events.
    • Effective communication with families during pediatric emergencies is crucial but challenging for prehospital providers.
    • There is a need for enhanced educational tools to support EMS providers in managing these difficult conversations.

    Purpose of the Study:

    • To pilot a novel cellular phone digital application (app) designed for EMS providers.
    • To assess the app's effectiveness in facilitating self-debriefing and improving communication skills after pediatric arrest events.
    • To evaluate the app's impact on team self-perception of communication abilities using simulation-based methodology.

    Main Methods:

    • A simulation-based study involving 148 EMS providers (38 groups) using two identical nonaccidental trauma simulations.
    • Intervention groups used a custom-developed app for team self-debriefing between simulations; control groups used standard debriefing methods.
    • Communication performance and self-assessment accuracy (gap analysis) were measured using the Gap-Kalamazoo Consensus Statement Assessment Form.

    Main Results:

    • No significant difference in initial communication performance between intervention and control groups (2.9/5 vs. 3.0/5).
    • The intervention group showed a statistically significant improvement in communication scores in the second assessment (4.0/5 vs. 3.1/5 control; p < 0.001).
    • The app significantly reduced the gap between perceived and actual communication abilities in the intervention group (0.04 vs. -0.8 control; p = 0.013).

    Conclusions:

    • An EMS-focused app can effectively guide providers in self-debriefing compassionate communication skills after critical events without a live facilitator.
    • The app significantly improves near-term communication patterns among EMS teams.
    • Group engagement with the app enhances the accuracy of team self-perception regarding communication abilities.
