Reflex Activity01:08

Reflex Activity

3.8K
A reflex activity is an automatic, involuntary response to specific stimuli. It is a part of our survival mechanism, designed to protect us from potential harm. For example, when a bright light suddenly shines into our eyes, we instinctively close them or look away. This is a simple reflex activity orchestrated by the nervous system without conscious thought or effort.

Focusing of Light in the Eye01:16

Focusing of Light in the Eye

6.9K
Light rays enter the eye through the cornea, a transparent dome-shaped tissue that is the eye's outermost layer. The cornea bends or refracts, light rays traveling to the pupil. The shape of the cornea determines how much of the light is bent and whether the image will be focused correctly on the retina at the back of the eye. Once the light has passed through both refraction layers, it converges into a single focal point onto a small area. This is where photoreceptors start transforming...

Blinding01:11

Blinding

4.0K
Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

Summary
This summary is machine-generated.

Fifty percent of patients undergoing septoplasty or septorhinoplasty exhibit a blink reflex during local anesthetic injection, even under deep general anesthesia. This common blink reflex does not necessitate deeper anesthesia and surgery can safely proceed.

Area of Science:

  • Anesthesiology
  • Otolaryngology
  • Surgical Procedures

Background:

  • Septoplasty and septorhinoplasty commonly involve local anesthetic infiltration.
  • A patient blink reflex during this infiltration has been anecdotally observed despite general anesthesia.

Purpose of the Study:

  • To prospectively quantify the incidence of the blink reflex during local anesthetic injection in patients undergoing septoplasty or septorhinoplasty.
  • To correlate the blink reflex with anesthetic depth and other physiological parameters.

Main Methods:

  • Prospective audit of patients undergoing septoplasty or septorhinoplasty.
  • Observation of blink reflex upon local anesthetic infiltration into the nasal vestibule.
  • Monitoring of anesthetic depth (propofol, remifentanil, bispectral index), vital signs, and patient movement.

Main Results:

  • A blink reflex was observed in 15 out of 30 patients (50%).
  • The average bispectral index was slightly higher in the blink group (32.75) compared to the non-blink group (26.77).
  • No patient movement was recorded during local anesthetic injection.

Conclusions:

  • The blink reflex occurs in approximately 50% of patients during nasal local anesthetic infiltration for septoplasty/septorhinoplasty, irrespective of deep anesthesia.
  • Awareness of this reflex is crucial to avoid unnecessary deepening of anesthesia.
  • Surgery can safely proceed despite the presence of the blink reflex.
