Summary

Thyrotoxic Periodic Paralysis (TPP) is a rare condition causing muscle weakness due to an overactive thyroid. Prompt treatment with potassium, beta-blockers, and anti-thyroid drugs can resolve symptoms and prevent recurrence.

Background:

  • It is characterized by proximal muscle weakness associated with thyrotoxicosis, predominantly affecting young Asian males.

Observation:

  • A previously healthy 35-year-old man presented with acute leg weakness after an alcohol binge.
  • Clinical assessment revealed tachycardia, urinary retention, and proximal limb weakness with preserved peripheral power.
  • Biochemistry demonstrated marked hypokalaemia and biochemical thyrotoxicosis.

Findings:

  • The patient was diagnosed with Thyrotoxic Periodic Paralysis (TPP) based on clinical and biochemical findings.
  • Intravenous potassium replacement, beta-blockers, and anti-thyroid therapy led to resolution of neurological symptoms.
  • Neurological recovery correlated with the patient becoming euthyroid.

Implications:

  • Acute management of TPP involves careful potassium supplementation, beta-blockade, and anti-thyroid therapy.
  • Maintaining euthyroidism is crucial for preventing TPP recurrence.
