Observation:

  • A 16-year-old male presented with esthetic and functional complaints related to a maxillary left lateral incisor, with a history of trauma to primary anterior teeth.
  • Clinical examination revealed altered esthetics and function, with positive response to percussion and negative pulp vitality test, indicative of pulpal pathology.
  • Cone-beam computed tomography (CBCT) confirmed tooth gemination, crown-root dilaceration, the presence of two root canals, and periapical bone rarefaction.

Findings:

  • Endodontic treatment involved instrumentation with Reciproc R40, irrigation with 1% sodium hypochlorite (NaOCl), and obturation using lateral condensation with gutta-percha and AH Plus sealer.
  • The treated tooth was restored with composite resin, successfully improving esthetics and function.
  • A 6-month follow-up demonstrated the absence of clinical symptoms and radiographic evidence suggesting apical healing.

Implications:

  • This case highlights the importance of CBCT in diagnosing complex dental anomalies like gemination with dilaceration and multiple root canals.
  • Successful endodontic management and restoration of geminated teeth can restore function and esthetics, improving patient quality of life.
