Evisceration for blind, painful eyes risks spreading unsuspected ocular tumors, leading to orbital recurrence and metastasis. Always send evisceration specimens for histology to enable prompt orbital radiotherapy if malignancy is found.

  • Undiagnosed ocular tumors can lead to tumor cell dissemination during evisceration.

Observation:

  • Four patients undergoing evisceration were found to have ocular tumors (choroidal melanomas, rhabdoid tumor, adenocarcinoma).
  • Two patients had a history of ocular trauma; two had prior treated choroidal melanoma.
  • Fundus examination was not performed pre-evisceration in 3 patients; imaging was inconclusive in one.
  • Three patients developed orbital recurrence and died; two had prior treated choroidal melanoma.

Findings:

  • Evisceration of eyes with unsuspected ocular malignancies is associated with poor prognosis.
