The formation of teeth, also known as odontogenesis, is a complex process that begins in utero, around the sixth week of embryonic development. There are three stages to this process: the bud stage, the cap stage, and the bell stage.

Purpose of the Study:

  • To systematically review the incidence and severity of root resorption associated with orthodontic aligner treatment.
  • To compare root resorption between aligner therapy and fixed multi-bracket treatments.
  • To identify factors associated with root resorption during aligner treatment.

Main Methods:

  • A systematic literature search was conducted across major databases (Medline, Embase, EBSCO, Cochrane, Science Direct) up to December 2015.
  • Included studies were meta-analyses, RCTs, non-randomized controlled trials, cohort studies, and descriptive studies.
  • Studies focused on root resorption incidence and severity with aligners, with comparisons to fixed appliances.

Main Results:

  • Only 3 studies met the inclusion criteria from 93 references.
  • Root resorption incidence with aligners ranged from 0% to 46% (6% severe).
  • Fixed multi-bracket treatment showed resorption incidence from 2% to 50% (22% severe) in non-extraction cases. Maxillary incisors were most affected in both groups, irrespective of age or sex.

Conclusions:

  • Orthodontic aligner treatment may be associated with a lower incidence of root resorption compared to fixed multi-bracket treatment in non-extraction cases.
  • Further investigation is required for extraction cases to fully evaluate root resorption with aligners.
  • Age and sex did not appear to influence root resorption incidence in the reviewed studies.
