Summary

Low-dose carperitide (α-human A-type natriuretic peptide) administration during oral surgery helped maintain higher hemoglobin levels. This suggests carperitide may reduce the need for blood transfusions in surgical patients.

Background:

  • Surgical stress activates the renin-angiotensin-aldosterone system (RAAS), causing fluid retention and reduced urine output (oliguria).
  • Carperitide, a natriuretic peptide, counteracts RAAS activation, promoting diuresis.
  • Previous observations suggest carperitide may mitigate decreases in hemoglobin (Hb) concentration during surgery.

Purpose of the Study:

  • To investigate the effect of low-dose carperitide (0.01 µg/kg/min) on hemoglobin concentration during prolonged oral surgery.
  • To assess the relationship between carperitide administration and perioperative fluid balance and diuresis.

Main Methods:

  • A randomized study involving patients (ASA-PS I-II, 40-80 years) undergoing oral maxillofacial surgery (>8 hours).
  • Patients received either low-dose carperitide (0.01 µg/kg/min) or normal saline (control).
  • Evaluated parameters included hemoglobin, urine volume, body fluid composition (TBW, ECW, ICW), PaO2, and serum electrolytes every 2 hours.

  • The carperitide group showed a smaller decrease in hemoglobin concentration compared to the control group (10.8 ± 1.5 g/dL vs. 9.5 ± 1.3 g/dL, p < 0.05).
  • Urine volume was significantly higher in the carperitide group (2557.3 ± 983.5 mL) versus the control group (1108.8 ± 586.4 mL, p < 0.001).
  • No significant differences were observed in body fluid volumes, PaO2, or electrolytes; no perioperative complications occurred.

Conclusions:

  • Low-dose carperitide administration maintained higher hemoglobin levels during surgery.
  • Carperitide may be a valuable adjunct for reducing blood transfusion requirements in surgical settings.
  • The findings support carperitide's role in managing fluid balance and mitigating intraoperative hemodilution.
