Summary

Off-pump coronary artery bypass surgery showed lower in-hospital mortality compared to on-pump surgery. Long-term survival rates were similar between the two coronary revascularization techniques.

Background:

  • Short-term outcomes of on-pump versus off-pump coronary revascularization are well-documented.
  • Long-term survival data comparing these two techniques remain limited.

Purpose of the Study:

  • To compare the long-term survival of patients undergoing off-pump coronary artery bypass grafting (CABG) versus on-pump CABG.
  • To analyze the incidence of cardiac events and mortality in both groups over an extended follow-up period.

Main Methods:

  • Retrospective analysis of 4687 patients who underwent coronary revascularization between November 1996 and March 2015.
  • Comparison of long-term survival and cardiac events between off-pump (n=3402) and on-pump (n=1285) groups.
  • Risk-adjusted analysis using propensity scores to mitigate bias, with a primary endpoint of all-cause mortality.

Main Results:

  • Off-pump surgery was associated with significantly lower in-hospital mortality (2.3% vs. 5.2%, p < 0.0001).
  • Long-term survival rates at 10 years were similar between off-pump and on-pump groups (77.9% ± 1.2% vs. 80.2% ± 1.3%, p=0.361).
  • Adjusted survival analysis also showed no significant difference in long-term survival between the two techniques (84.2% ± 2.9% vs. 80.3% ± 2.4%, p=0.169).

Conclusions:

  • Off-pump coronary artery bypass surgery is associated with reduced in-hospital mortality.
  • The study did not find evidence of increased long-term survival for off-pump compared to on-pump coronary revascularization.
