Tremor is more common in progressive supranuclear palsy (PSP) than previously thought, occurring in 12-16% of cases. A slow resting tremor can be a presenting symptom, complicating differential diagnosis from Parkinson's disease.

Area of Science:

  • Neurology
  • Movement Disorders

Background:

  • Progressive supranuclear palsy (PSP) is a neurodegenerative disease.
  • Tremor is considered infrequent or absent in PSP by some researchers.

Observation:

  • This study presents five cases of PSP with tremor.
  • Literature review suggests tremor occurs in 12-16% of PSP cases.

Findings:

  • A slow resting tremor (≤3 Hz) was observed in three cases, differing from typical Parkinsonian tremor (4-6 Hz).
  • This tremor can be a presenting and prominent symptom in PSP.

Implications:

  • The presence of tremor does not exclude PSP.
  • Tremor characteristics may aid in differentiating PSP from Parkinson's disease.
