Background:

  • A 25-year-old male presented with right heel pain and swelling after a fall.
  • Physical examination revealed a tender, soft swelling on the medial aspect of the heel.
  • Imaging confirmed a cystic lesion in the calcaneum.

Findings:

  • The patient underwent curettage and bone grafting for the calcaneal lesion.
  • Postoperatively, the patient experienced pain relief and full weight-bearing capacity within 12 weeks.
  • Histopathological analysis confirmed the diagnosis of aneurysmal bone cyst.

Implications:

  • Aneurysmal bone cyst of the calcaneum, though rare, must be considered in the differential diagnosis of cystic heel lesions.
  • Curettage and bone grafting remain a reliable and effective treatment for calcaneal ABCs.
  • This case underscores the importance of considering rare presentations of common bone pathologies.
