Background:

Lichen striatus is a rare, acquired skin condition characterized by papules and plaques arranged linearly.
Understanding the histopathologic features of lichen striatus is crucial for accurate diagnosis and management.

Purpose of the Study:

To classify the histopathologic changes observed in skin biopsies of patients with classical lichen striatus.
To determine the prevalence of different histopathologic patterns in lichen striatus.

Main Methods:

Skin lesions from 49 patients with classical lichen striatus were biopsied.
Histopathologic examination of the biopsy specimens was performed.
Changes were classified into distinct dermatopathologic patterns.

Main Results:

The most common histopathologic pattern observed was perivascular dermatitis (51%).
Perivascular and periappendageal dermatitis (26.5%) and lichenoid dermatitis (22.5%) were also identified.
Three specimens additionally showed islands of epithelioid cells.

Conclusions:

Perivascular dermatitis is the predominant histopathologic finding in classical lichen striatus.
The histopathologic spectrum of lichen striatus includes perivascular, periappendageal, and lichenoid patterns.
These findings contribute to the histopathologic characterization of lichen striatus.
