Summary

The chemical shift effect (CSE) on MRI reliably predicts lymph node status in rectal cancer patients. While CSE is highly accurate, combining it with other criteria like nodal border and signal intensity improves diagnostic confidence.

Background:

Purpose of the Study:

  • To assess the diagnostic value of the chemical shift effect (CSE) and other MRI criteria for predicting lymph node metastasis in rectal cancer.
  • To compare the efficacy of CSE against traditional nodal assessment criteria.

Main Methods:

  • Preoperative and postoperative specimen MRI were performed on 29 rectal cancer patients.
  • Lymph nodes were precisely matched between whole-mount specimens and MRI images.
  • Two independent readers evaluated preoperative MR characteristics, including CSE, nodal location, border, signal intensity, and distance to the rectal wall.

  • The chemical shift effect (CSE) demonstrated high accuracy in differentiating benign from metastatic lymph nodes (AUCs of 0.879 and 0.845).
  • Absence of CSE was significantly associated with metastatic nodes (80.0% and 74.3% detection rates).
  • Other criteria like nodal location and border were useful but less accurate than CSE (AUCs 0.629-0.743).

Conclusions:

  • CSE is a highly reliable MRI predictor for lymph node status in rectal cancer.
  • While CSE is effective, incorporating additional criteria enhances diagnostic certainty, especially in ambiguous cases.
  • Nodal border, signal intensity, and location are valuable supplementary predictors.
