Summary

Natchev's auto-traction (NAT) offers a convenient and effective treatment for chronic lumbago-sciatica caused by lumbar disc herniation. This new method shows comparable efficacy to Lind's auto-traction (LAT) with improved patient and therapist experience.

Area of Science:

  • Orthopedics
  • Physical Medicine & Rehabilitation
  • Neurosurgery

Background:

  • Lind's auto-traction (LAT) is a treatment for lumbago-sciatica due to lumbar disc herniation, but it requires significant therapist effort and patient inconvenience.
  • A modified version, Natchev's auto-traction (NAT), was developed to address these limitations.

Purpose of the Study:

  • To evaluate the effectiveness of Natchev's auto-traction (NAT) in patients with chronic lumbago-sciatica refractory to prior treatments.
  • To compare NAT's outcomes with historical data from LAT and assess its suitability as a routine or screening treatment.

Main Methods:

  • An open prospective trial was conducted on 77 patients with confirmed lumbar disc herniation.
  • Patients received NAT in 3-10 half-hour sessions.

Related Experiment Videos

  • Outcomes, including pain reduction and need for surgery, were assessed at 6 months post-treatment.
  • Main Results:

    • 47% of patients (36/77) responded to NAT, with a median pain reduction to 27% of pretreatment levels.
    • Among responders, 28/36 maintained stability at 6 months, with only 4 undergoing surgery.
    • Non-responders had a higher rate of surgery (20/41).

    Conclusions:

    • NAT is as effective as LAT for treating lumbar disc herniation-related lumbago-sciatica.
    • NAT's convenience makes it a suitable routine treatment and a potential pre-surgical screening tool.
