Summary

The uncinate duct, embryologically the main pancreatic duct of the ventral pancreas, may be a distinct subtype of intraductal papillary mucinous neoplasm (IPMN). Isolated uncinate-duct IPMN might require management similar to main-duct IPMN due to malignancy risk.

  • Current intraductal papillary mucinous neoplasm (IPMN) classifications do not fully address the uncinate duct's embryologic origin.

Observation:

  • Intraductal papillary mucinous neoplasms (IPMNs) are classified based on ductal involvement (main-duct vs. branch-duct).
  • Main-duct IPMNs are considered high-risk for malignancy, while branch-duct IPMNs are typically low-risk.
  • Isolated uncinate-duct IPMN presents a unique classification challenge.

Findings:

  • Embryologically, the uncinate duct represents the main pancreatic duct of the ventral pancreas.
  • Isolated uncinate-duct IPMN shows an increased association with high-grade dysplasia and invasive cancer.
  • This suggests uncinate-duct IPMN may share malignant potential with main-duct IPMN.

Implications:
