This case highlights a rare cardiac complication in ankylosing spondylitis (AS). We report an association between AS and non-compaction cardiomyopathy, a condition affecting heart muscle function.

- It suggests that LVNC should be considered in the differential diagnosis of cardiomyopathy in AS.
