Background:

Granuloma inguinale (donovanosis) is a chronic bacterial infection.
It can present with extensive ulcerative lesions that may mimic malignancy.
A history of urethritis and fistula formation was noted in this patient.

Observation:

The patient presented with extensive granuloma inguinale lesions.
The lesions were clinically suggestive of squamous cell carcinoma.
Associated conditions included scabies and reactive VDRL (syphilis).

Findings:

Streptomycin therapy resulted in near-complete healing of the ulcer within 19 days.
The VDRL test indicated a reactive result (1:8 dilution), suggesting possible latent syphilis.
The patient was from Madhya Pradesh, an endemic region.
